Please follow the instructions below to place your order: 

• Enter your pet's name, your phone number/address to be included on the back of the tag
• Enter any additional information for the back of the tag/special instructions (eg: medical conditions, vet info)
• Choose Size: Large or Small (small is for pets less than 20 pounds)
• Enter any custom requests you may have - specific colours, designs, layouts.  We are happy to provide a proof if requested as well
