He is missing an eye, has not been properly groomed in a long time, and is extremely fearful and shy.

His eye was an old injury that has now healed, and we may never know how he lost it. Thankfully, there aren't currently signs of infection, but he still needs regular medical attention, grooming, food, vaccinations, and care.
