• Third-degree uterine prolapse — primary indication; effective for advanced prolapse where ring pessary has been unsuccessful but some introital support is present
• Cystocele — anterior vaginal wall prolapse (bladder herniation); the inflatable donut's volumetric fill provides broad anterior compartment support
• Rectocele — posterior vaginal wall prolapse (rectal herniation); donut provides posterior compartment contact and support
• Patients where ring pessary has failed — the inflatable donut is a step-up option when a ring cannot provide sufficient support
• Patients with some remaining introital support — the inflatable donut requires some introital tone to remain in position; it is not indicated for patients with no introital support (cube pessary may be more appropriate in that case)
• Patients who can manage frequent self-removal — the 48-hour wear interval requires the patient or caregiver to remove and reinsert the pessary regularly

How It Works — Inflatable Donut vs. Standard Donut

The Inflatable Donut Pessary is a silicone evolution of an older latex device called the Inflatoball — updated in both material and mechanism for modern clinical use. Like the standard donut pessary, it functions as a thicker, more volumetric support structure than a ring, filling a vaginal vault enlarged by loss of elasticity more completely and providing greater pelvic floor contact across the anterior and posterior compartments. Where it differs critically from a standard solid donut is in its inflation mechanism: the pessary has a stem with a valve, allowing it to be partially or fully deflated for insertion and removal, then inflated in situ to achieve its full support diameter.

This inflation mechanism solves the primary practical challenge of the standard solid donut — introital passage. A solid donut requires firm digital compression and significant lubrication to pass through the introitus, which can cause discomfort and limits the patient population able to self-manage. The inflatable version can be passed through the introitus in a deflated or partially deflated state, significantly reducing insertion difficulty, then inflated via the stem valve to fill the vault. Prior to removal, the pessary is deflated, making extraction considerably easier than removal of a fully inflated or solid donut. This design is intended for frequent insertion and removal cycles — within 48-hour intervals — rather than extended continuous wear.
